This programme takes a fundamentally different approach, tracking crown lengthening as a complete, unbroken clinical sequence:
At the heart of this sequence is the Active Restorative Loop. Dr Malik is a Specialist in Prosthodontics with nearly 13 years of experience in complex restorative, aesthetic and implant dentistry. He holds a clinical academic position at the UCL Eastman Dental Institute, where he teaches and supervises postgraduate dentists in both research and advanced clinical practice relating to fixed and removable prosthodontics.
His work focuses on challenging cases, including full mouth rehabilitations, advanced tooth wear, developmental anomalies (such as hypodontia and amelogenesis imperfecta) as well as the prosthodontic management of oncology patients.
His courses combine academic rigour with clinical relevance, and are designed for dentists who want to deepen their understanding of restorative, implant, and digital workflows.
He has received multiple awards, including First Prize at the Italian Academy of Prosthodontics (AIOP), the UCL Eastman 3M Prize for Clinical and Academic Excellence, and recognition from the British Society of Prosthodontics (BSSPD).
Dr. Khurrum Hussain is a highly experienced Specialist Periodontist and Implant Surgeon with extensive expertise in managing complex periodontal and implant cases. Recognised on the General Dental Council’s Specialist List for Periodontics, he has built a reputation for his precision in cosmetic gum surgery, mucogingival procedures, and advanced implant rehabilitation aimed at restoring both oral health and aesthetics.
Alongside his clinical practice, Dr. Hussain plays an active role in postgraduate education. He serves as a Clinical Educator at the University of Manchester Dental Hospital, where he mentors and trains dentists in surgical and non-surgical periodontics, implantology, and regenerative techniques.
Dr. Hussain is also the co-founder of PerioMatters, an educational platform dedicated to advancing periodontal and implant knowledge among clinicians through courses, discussions, and evidence-based training. His commitment to professional development extends to his position on the leadership team of the National Osteology Group UK, where he contributes to bridging the gap between scientific research and clinical application.

Day 1 establishes the biologic rules of crown lengthening and introduces the restorative philosophy of the course from the very first session. The focus is on understanding tissue attachment, tissue architecture, smile design, and the direct relationship between surgical alteration and restorative design. Vertical preparation mechanics are introduced early so delegates understand that margin design is fundamentally intertwined with healing biology.

• Using tissue attachment parameters as the primary starting point for restorative planning.
Session 2
The Vertical Shift
• Biomechanical comparison of horizontal (shoulder/chamfer) versus vertical margin concepts.
• Why fixed horizontal finish lines create chronic tension and inflammatory loops beside a healing surgical site.
• The Vertiprep paradigm: Gaining restorative freedom, margin flexibility, and structural thickness during post-surgical healing.

Aesthetic Diagnosis and Altered Passive Eruption

1. Diagnostic Bone Sounding and Flap Design Simulation: Phenotype and bone architecture assessment on clinical models; precision incision planning and soft-tissue versus osseous decision-making.
2. Aesthetic Crown Lengthening on Typodonts:Inverse bevel incision execution, full and split-thickness flap reflection, ostectomy/osteoplasty, and advanced suturing techniques for controlled aesthetic margin positioning.
3. Digital Architecture Workshop: Proportion planning using presentation software; translating macro-aesthetic smile analysis into micro-surgical design parameters.
The Compromised Tooth, Vertipreps and the Active Restorative Loop
Theory
Day 2 focuses on functional crown lengthening and the management of the severely broken-down tooth. This is where surgery and restoration are fully joined. The restorative side of the course is no longer presented as an afterthought, but as an active restorative loop in which provisional vertical restorations dynamically guide the tissues through healing toward an aesthetically and biologically stable final outcome.

The Compromised Tooth
• Managing structural loss, subgingival defects, subcrestal fractures, and perforations.
• Objective restorability assessment and determining the strategic value of the tooth.
• Ferrule requirements and engineering resistance form in high-load areas.
• Surgical crown lengthening vs. orthodontic extrusion vs. surgical extrusion: Critical decision-making.
Session 2
The Ferrule Equation
• Ferrule as a dynamic biomechanical requirement, not just a static measurement.
• Margin positioning, root trunk access, and managing the surgical furcation risk.
• Quantifying exactly how much sound tooth structure must be exposed intra-operatively.
• Identifying when a tooth remains too compromised even after successful surgical exposure.
Session 3
The Active Restorative Loop (The Masterclass Timeline)
• Phase 1: Pre-Op Engineering:Defining the restorative goal, assessing remaining tooth structure, selecting the vertical preparation pathway, and fabricating provisionals before the blade touches tissue.
• Phase 2: Surgical Execution: Creating predictable osseous relief, establishing the required supracrestal space, and preparing the root architecture for margin adaptation.
• Phase 3: The 7 to 10 Day Tissue Check: Suture removal, early tissue review, and initial provisional relines (Provisional On-Growth/Conditioning) to protect the healing clot interface.
• Phase 4: The 3-Week Dynamic Prep: Reassessing tissue position and utilizing the freedom of a vertical preparation to alter margin depth without removing further tooth structure.
• Phase 5: 6 Weeks to 3 Months (Tissue Conditioning): Using highly polished provisional emergence profiles to actively sculpt, guide, and support the free gingival margin rather than waiting passively.
• Phase 6: The 3 to 9 Month Finalisation Window: Identifying true histological tissue maturity across different phenotypes and delivering the final restoration with absolute positional confidence.

Vertiprep Masterclas

2. Vertiprep and Provisionalisation Lab:Executing a precise vertical preparation on a structurally compromised typodont tooth; provisional crown fabrication, anatomical relining, and micro-polishing designed to actively condition a healing gingival margin.
